I hereby give permission to my student, to participate in all NTS activities and program events. To spend the week at The Rustic Inn.

In the case of an emergency, I do not hold First Position, staff, other students or location owners any injuries, sickness, lost, stolen or damaged items.

Photos will be taken and used only for promotional purposes.

I accept payment for the training of $400.00 and understand that payment must be sent in before my student can be added to attendance list. I understand that if payment isn't sent in before max capacity is reached that my student may end up on a waiting list.
